Our Case Managers are strong client advocates, often educating non-recovery service providers about recovery and the specific needs of the client, whose best interests are always put first.

As the single point of contact, our Case Managers begin their specialized care by meeting the clients “where they are,” making sure that basic necessities like food, shelter, utilities, transportation, child care are met. Case Managers often coordinate with clients’ families, friends, churches, organizations and self-help groups to ensure that their personal spiritual and emotional needs are being integrated into the client’s care.
For institutionalized clients, Case Management may involve preparing these clients for services post-discharge to ensure an uninterrupted continuity of services. Such services include extended care treatment or living within their local communities. Case Managers help ensure that all transitions between the various service providers are smooth and that any obstacles are easily identified and overcome.
